Target Audience
Physicians, scientists and health care professionals involved in providing renal replacement therapy and other forms of blood purification
Objectives
Participants will become familiar with the latest developments in the science of blood purification and the management of renal failure.
Online Registration
Registration procedure
All participants must be registered via the website or by filling in the registration form included in the brochure of the meeting. Upon receipt of your registration form and payment or credit card details, the congress secretariat will confirm your participation in the meeting. You will receive a formal letter of confirmation with additional information about the meeting. Early registrations are due by May 30, 2005. The registration fee for participants includes the following: Admission to all scientific sessions, all congress materials including the final programme, abstract book, administration costs, coffee breaks, lunch(es) and the welcome reception on August 31, 2005. The registration fee for accompanying persons includes the following: Welcome reception on August 31, administration costs and lunch(es).
Payment details
All payments must be made in Euro. Payments should be made in advance using one of the following methods: Money order made payable to (you will receive an invoice): Het Congresbureau Erasmus MC ABN/AMRO Bank Rotterdam, The Netherlands Account number: 49.69.70.933. Visa, Euro/MasterCard Admission to the meeting is granted only if the congress secretariat has received the registration fee. Participants who have made late payments should bring a copy of their proof of payment (copy of bank transfer) to the registration desk in Rotterdam.
Cancellation and refunds
Should you be unable to attend the meeting, a substitute delegate is welcome at no extra charge. In this case a letter of authorisation, mentioning the name of the substitute delegate, should be provided by you (the original participant) before the starting date of the meeting. All cancellations should be in writing (letter/fax) and send to the congress secretariat. In case of cancellation (in writing) before or on July 15, 2005, you will have to pay 25% of the total amount (the registration fees, social events etc.) you are registered for. For cancellations after July 15, 2005, unfortunately there will be no refund and the full amount will remain due.
